Log in to UDID portal using "PwD Login" Tab and select 'Renew UDID Card.' Submit your request for renewal of existing UDID card. After successful submission of the request, the applicant shall have to follow the instructions given by the Hospital for disability assessment which is necessary for issuance of renewed UDID card.
