The Disability certificate and UDID card is issued to eligible person based on the disability assessment by the Hospital. The issuance of disability certificate and UDID card is subjected to provisions given in the disability assessment guidelines no. 1338 (E) dated 12.03.2024. The disabililty certificate and UDID card is issued after following up of the due process.
