The disability certificate and UDID card is issued to eligible person based on the disability assessment by the Hospital. The issuance of disability certificate and UDID card is subjected to provisions given in the disability assessment guidelines no. 1338 (E) dated 12.03.2024. In case staff are not cooperating, you can raise your grievance at the designated authority at the hospital or the matter may be escalated with higher authority in the hospital or Directorate General of Health Services/or Health Department  of the State/UT.
