The details of the Hospital where the disability assessment would take place is also available on the acknowledgment slip. In case issuance of UDID card and disability certificate is delayed by the Hospital without justified reasons, the matter may be escalated with higher authority in the hospital or Directorate General of Health Services/or Health Department of the State/UT.
