The disability certificate and UDID card is issued by the designated hospital notified by the State/UT Government. It is advised to visit the hospital that you have selected at the time of submission of your application. The details of the Hospital where the disability assessment would take place is also available on the acknowledgment slip. In case issuance of UDID card and disability certificate is delayed by the Hospital without justified reasons, the matter may be escalated with higher authority in the hospital or Directorate General of Health Services/or Health Department of the State/UT.
